Information on the prior evaluation of tenders referred to in Article 139 of the Public Procurement Act: The Contracting Authority informs that pursuant to Art. 1 of the Public Procurement Act, will first examine and evaluate the tenders, and then make a qualitative qualification of the Contractor whose tender was the highest rated, as regards the absence of grounds for exclusion and compliance with the conditions for participation in the procedure. 2. Together with the tender, the Contractor must submit: the European Single Procurement Document; Declaration by the economic operators jointly applying for the contract (if any); Commitment of the provider of resources (if any); Declaration by the contractor(s) jointly applying for the contract on the separate grounds for exclusion; a declaration by the provider of the resources on the separate exclusion grounds; f) A document showing the scope of the authorization to act on behalf of the Contractor in the procurement procedure; g) The means of proof in question : Product brochure/product catalogue/technical specification of the offered devices confirming the fulfilment of all parameters specified by the contracting authority in the tender specifications (in Polish) - the contracting authority envisages supplementing the means of proof referred to in the above point. Before selecting the most advantageous tender, the Contracting Authority shall request the Contractor whose tender has been awarded the highest score to submit, within a specified period of not less than 10 days, up to the date of submission, the following subjective means of proof or the Contractor’s certificate replacing the relevant subjective means of proof, within a period of not less than 10 days from the date of the request. The certificate will be recognised by the Contracting Authority only to the extent that it confirms the absence of grounds for exclusion or fulfilment of the conditions for participation in the procedure set out in this tender specification: 1) In order to confirm that the Contractor meets the conditions for participation in the procedure: a) A document confirming that the Contractor is insured against civil liability in respect of activities related to the subject of the contract for a guarantee amount not less than PLN 400 000.00 (in words: four hundred thousand zlotys); b) List of supplies and services; c) List of persons; d) Certificate or equivalent attestations attesting to ISO 9001 and 27001 standards for at least cybersecurity-related services; 2) In order to confirm the absence of grounds for exclusion of the Contractor from participation in the procedure: a) Information from the National Criminal Register; b) Statement of the contractor on the capital group; c) Copy or information from the National Court Register or CEIDG; d) Certificate from ZUS or KRUS; a declaration by the contractor that the information contained in the initial declaration is up-to-date; f) Statement by the provider
