HIS InfoMedica Plus/AMMS Plus application software service

… declares that the software license provider is Asseco Poland S.A. The Contractor will service the HIS InfoMedica Plus/AMMS Plus application software within 24 months at the earliest from 01.02.2027. 1. The scope of activities performed as part of the InfoMedica Plus /AMMS Plus system service: a. Installing and implementing current versions of the application software; b. AMMS Plus system updates will be performed on the date agreed by the parties to the agreement. The Customer undertakes to download from the software manufacturer's ftp all the files necessary to perform the update. The update will be carried out with the cooperation of an IT employee on the part of the Ordering Party in a manner and at a time that does not interfere with the Ordering Party. c. Adjusting the configuration of the application software to the needs of the Ordering Party, inter alia, assistance in defining templates, reports, dictionaries, forms, lists, etc. d. Providing instructions, if necessary, in the preparation of various reports, reports, etc. e. Advice on the preparation of data provided by the Ordering Party to parent and cooperating entities (e.g. to the National Health Fund, the Health Department of the relevant Office, banks, etc.) in electronic form (e.g. floppy disks, telecommunications links, etc.). f. Supplementary consultations, assistance to users in using the application software. Consultation and training may be conducted remotely at the indicated user stations and by telephone. g. Consultancy and assistance in administering the Oracle database. h. Performance tuning and optimization of database software configuration taking into account the needs of the Ordering Party and the requirements of application software. i. Ongoing optimization of software configuration taking into account the needs of the Ordering Party. j. Assistance in emergency reconstruction at the request of the Ordering Party of the state of the software and archival data collected, properly secured by the Ordering Party on appropriate media. k.
