… areas in Nowy Tomyśl a) The subject of the contract is the maintenance of lawns and green areas and associated works within the boundaries of District II. The scope includes the areas of the Północ housing estate, the Batory housing estate and the adjacent green areas listed in the annexes. (b) A detailed description of the subject of the contract is contained in the documentation constituting Annex 1b to the tender specification. 2. In accordance with Art. 1 PZP The Contracting Authority shall invite the economic operator whose tender was the highest rated to submit, within a specified period of not less than 10 days from the date of the request, subjective evidence, if it required it to be submitted in the contract notice or the procurement documents, up to the date on which the subjective evidence was submitted. 3. The contracting authority shall make use of Article 139(1)(b) CTMR. 1 PHP, i.e.: The contracting authority will first examine and evaluate the tenders and then make a qualitative selection of the economic operator whose tender was ranked the highest in terms of the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure. 4. The contractor may submit only one tender per lot. The Contracting Authority accepts the submission of partial tenders 5. Summary of documents that make up the tender: 1) Completed offer form (according to the template – Annex 2 to the tender specification), 2) ESPD statement; 3) the Contractor's statement regarding the grounds for exclusion in Article 5k of Regulation 833/2014 4) a copy or information from the National Court Register or the CEiDG, if separate provisions require entry in the register or register, in order to confirm that a person acting on behalf of the Contractor is legally entitled to represent him; 5) confirmation of the deposit. In addition, the tender must be accompanied, if applicable, by: 6) power of attorney authorising to submit an offer, if the offer is signed by a representative; 7) power of attorney for a representative to represent in the procedure Contractors jointly applying for the contract – concerns tenders submitted by Contractors jointly applying for the contract, 8) obligation of a third party; 9) a statement of the ESPD for each of the entities making resources available to the Contractor, if the Contractor relies on the resources of other entities; 10) a statement of the entity making resources available regarding the grounds for exclusion from Article 5k of Regulation (EU) No 833/2014 (if applicable); a statement of the ESPD for each of the economic operators jointly applying for the award of the contract, in the case of economic operators jointly applying for the award of the contract; 12) a statement concerning the grounds for exclusion under Article 5k of Regulation (EU) No 833/2014 for each of the economic operators jointly applying for a contract, in the case of economic operators jointly applying for a contract; 13) a statement pursuant to Art. 4 of the Public Procurement Act referred to in Chapter X par. 6 SWZ (if applicable). · 1. The subject of the contract is year-round provision of services for ongoing cleaning, winter maintenance and associated works within the boundaries of District I. The scope includes the city of Nowy Tomyśl, excluding areas assigned exclusively to District II and indicated objects and routes in rural areas. 2. A detailed description of the subject-matter of the contract is contained in the documentation constituting Annex 1a to the ToR.
