Участниците следва да отговарят на изискванията на чл. 77, ал. 1 от Закона за медицинските изделия и да притежават валидно разрешение за търговия на едро с медицински изделия, съгласно чл. 77 от Закона за медицинските изделия, издадено от Изпълнителната агенция по лекарствата или друг документ, удостоверяващ правото им да търгува с медицински изделия, издаден от компетентен орган на съответната държава – държава членка на Европейски съюз или друга държава–страна по Споразумението за Европейското икономическо пространство, или на Конфедерация Швейцария. Изискването за представяне на разрешение/документ по чл. 77 от ЗМИ не важи за производителите на съответните медицински изделия, установени на територията на Република България, които могат да извършват търговски сделки с произведените от тях изделия без документа по чл. 77, ал. 1 от ЗМИ. При подаване на офертата участникът декларира наличието на поставеното от Възложителя изискване за годност в част IV, Раздел А от ЕЕДОП. Ако съответните документи са на разположение в електронен формат, участниците могат да посочат уеб адрес, орган или служба, издаващи документа, точно позоваване на документа. Staff qualifications

За изпълнение на поръчката участниците следва да имат на разположение най-малко едно техническо лице - сервизен специалист, отговорен за сервиз и гаранционно обслужване на предлаганото оборудване, който следва да е преминал курс на обучение при производителя или упълномощен от него представител за сервиз и гаранционно обслужване на предлаганото оборудване или да има професионален опит като сервизен специалист по поддръжка на медицинска/дентална апаратура минимум 2 /две/ години. Участникът следва да декларира съответствието си с поставения критерий за подбор, като представи информация за обстоятелствата в част ІV, раздел В от ЕЕДОП под формата на списък на техническите лица, които ще са ангажирани с изпълнението на поръчката като се посочи следната информация: Име и фамилия на лицето/лицата; идентификационни данни от сертификати или удостоверения за преминато обучение и квалификацията на предложения сервизен специалист (ако е приложимо) или професионален опит като сервизен специалист (период, месторабота, длъжност) на предложеното лице, както и оборудването за чиито сервиз и поддръжка е ангажиран. За доказване на съответствието с минималното изискване при условията на чл. 67, ал. 5 от ЗОП възложителят може да изиска от участника допълнително и по всяко време документи, доказващи придобитата компетентност на посочения/те в ЕЕДОП от участника сервизен/сервизни специалист/и. При условията на чл. 112, ал. 1, т. 2 от ЗОП, преди сключване на договора, участникът, определен за изпълнител представя документите по предходното изречение (ако не са били представени по реда на чл. 67, ал. 5 от ЗОП).

Independent quality certificate (e.g. ISO 9001)

Участникът следва да прилага внедрена система за управление на качеството по стандарта БДС EN ISO 9001 или еквивалентен с обхват на действие внос, търговия и/или сервиз на дентална апаратура/медицински изделия/апаратура екв. сертификат или други доказателства за еквивалентни мерки. Сертификатът трябва да е валиден и издаден от независими лица, които са акредитирани от Изпълнителна агенция “Българска служба за акредитация” или от друг национален орган по акредитация, който е страна по Многостранното споразумение за взаимно признаване на Европейската организация по акредитация, за съответната област или да отговарят на изискванията за признаване съгласно чл. 5а, ал. 2 от Закона за националната акредитация на органи за оценяване на съответствието. Възложителят признава еквивалентни сертификати, издадени от органи, установени в други държави членки. Когато участникът не е имал достъп до този сертификат или е нямал възможност да го получи в съответните срокове по независещи от него причини, той може да представи доказателства за еквивалентни мерки за осигуряване на системата за управление на качеството. В тези случаи участникът трябва да е в състояние да докаже, че предлаганите мерки са еквивалентни на изискваните. Участникът следва да декларира изпълнението на поставеното от Възложителя изискване, като попълни част ІV, раздел Г от ЕЕДОП. Ако съответните документи са на разположение в електронен формат, участниците могат да посочат в ЕЕДОП: уеб адрес, орган или служба, издаващи документа и точно позоваване на документа.
