… institutions. The Contractor must indicate the number of establishments in the list annexed to the tender form. Failure to comply with this requirement will result in the rejection of the tender. The contracting authority provides for the possibility of exercising the right of option. Primary procurement: Provision of medical services in the field of additional medical care for employees of the company Applications Krytyczne Sp. z o.o. (Ordering Party) and their families for a period of 36 months from the date of signing the contract. Contract under option law: Provision of medical services in the field of additional medical care for employees of the company Applications Krytyczne Sp. z o.o. (Ordering Party) and their families for a maximum period of 12 months from the date of termination of the contract. 2. A detailed description of the subject-matter of the contract is contained in Annex 1 to the ToR – Description of the subject-matter of the contract (OPZ).
