… applicable regulations in the field of the Atomic Law Act and all related regulations of the Ministry of Health (including checking the image quality, checking the measurement and application values of the apparatus using, if necessary, specialized measuring equipment and phantoms) • Performing acceptance and specialist tests in accordance with the currently applicable law and communicating the results to the Ordering Party • Inspection of the device after repair • Review of the Optima 660 CT type computer tomography in accordance with the manufacturer's guidelines • Documentation of service intervention 3. A detailed description of the subject-matter of the contract is set out in the following annexes: 1) Price form – Annex 1.1 to the ToR, 2) Draft provisions of the contract – Annex 2 to the ToR. 4. The contracting authority does not allow for the possibility of submitting partial tenders. 5. The subject of the contract is not divided into lots: Justification: the division of the contract into lots would generate increased costs for the performance of the subject-matter of the contract due to the fact that the subject-matter of the contract is the supply of two components compatible with each other and necessary for the commissioning of the equipment. The division is also unjustified due to the conditions of the guarantee, which in the case of two different contractors would be difficult to enforce due to the fact that the contract is also covered by the element of commissioning of the equipment after the replacement of both elements, i.e. the x-ray tube and the control plate. The Contracting Authority informs that pursuant to Article 139 of the PPL, it will first examine and evaluate the tenders, and then the qualitative selection of the Contractor whose tender was the highest rated in a given package, as regards the absence of grounds for exclusion and compliance with the conditions for participation in the procedure. In fulfilling the obligation under Art. 6 of the Act of 14 June 2024 on the protection of whistleblowers, the contracting authority informs that the procedure for internal notifications of information on breaches of law and for follow-up actions has been adopted at the Independent Public Health Care Institution of the Provincial Specialist Hospital No 3 in Rybnik.
