… of the Public Procurement Act consisting in the repetition of similar services within 3 years from the date of award of the basic contract, in the event of an increase in the scope of the provision of medical services for the purposes of preventive (preliminary, periodic and control) tests for soldiers and employees of the Ministry of National Defence of the 28th Military Economic Branch and units and institutions that are on its supply, in the amount of up to 70% of the basic contract. The award of the contract will take place on the terms of the basic contract and will require the conclusion of a separate contract. The subject of the contract is the provision of medical services for the purposes of preventive tests (preliminary, periodic, control) for soldiers and employees of the Ministry of National Defence of the 28th Military Economic Branch and units and institutions that are on its supply in 2027, located in the towns of Siedlce, Garwolin (according to the list indicated in Annex 1 to the ToR). The requirements and a detailed description of the subject-matter of the contract are set out in Annex 1 to the tender specifications (Description of the subject-matter of the contract) and in Annex 6 to the tender specifications (draft provisions of the contract and annexes) and in the other tender documents, which will be published on the website of the ongoing procedure https://platformazakupowa.pl/pn/28wog. The Contracting Authority shall provide for the possibility of awarding contracts to the Contractor pursuant to Article 214(1)(b) CTMR. 1 point 7 of the Public Procurement Act consisting in the repetition of similar services within 3 years from the date of award of the basic contract up to 70% of the basic contract. The award of the contract will take place on the terms of the basic contract and will require the conclusion of a separate contract. The Contractor is obliged to provide services in an establishment located no more than 60 km from the seat of the Ordering Party, i.e. 28 WOG in Siedlce (ul. Pilecki 5). The subject of the contract is the provision of medical services for the purposes of preventive examinations (preliminary, periodic, control) for soldiers and employees of the Ministry of National Defence of the 28th Military Economic Branch and units and institutions that are on its supply in 2027, located in Warsaw-Wesoła (according to the list indicated in Annex 1 to the ToR).
