… conjunction with Article 132 et seq. of the Public Procurement Law Act of 11 September 2019, applicable to classic contracts with a value equal to or exceeding the EU thresholds, therefore the Contracting Authority, using the powers enshrined therein: 1) instead of the single document form (JEDZ), requests that the Contractor attach to the tender a declaration of non-exclusion and compliance with the conditions for participation – Article 125(1) of the Public Procurement Act 2) shortens the minimum time limit for submitting tenders to 15 days, pursuant to Article 138(1) CTMR. 2 point 2 of the Public Procurement Act, because there is an urgent need to award the contract. 2. The subject of the contract are the services listed in Annex XIV to Directive 2014/24/EU of the European Parliament and of the Council of 26 February 2014 on public procurement and repealing Directive 2004/18/EC. Names and codes according to the Common Procurement Vocabulary: 85000000- 9 health and social care services 85311200-4 social care services for people with disabilities.
