The Employer requires the employment by the contractor or subcontractor on the basis of an employment contract of a person(s) performing activities in the scope of contract performance, the performance of which consists in performing work in the manner specified in Article 22 § 1 of the Act of 26 June 1974 - Labour Code (Journal of Laws No. Journal of Laws of 2025, item 277 as amended), i.e. the employment of persons who will perform activities directly related to the provision of credit granting services, i.e.: launching a loan in the banking system, calculating bank interest due, informing about the current state of the loan, unless these activities are carried out by persons in the course of their business activity. 3. In order to verify the employment, by the contractor or subcontractor, on the basis of an employment contract, of persons performing the activities indicated by the contracting authority in the field of contract performance, the contracting authority provides for the possibility of requesting, in particular: 1) statements of an employed employee, 2) statements of the contractor or subcontractor on the employment of an employee on the basis of an employment contract, 3) certified true copy of the employment contract of an employed employee, 4) other documents - containing information, including personal data, necessary to verify employment on the basis of an employment contract, in particular the name and surname of the employed employee, the date of conclusion of the employment contract, the type of employment contract and the scope of the employee's duties. 4. Copies of the documents referred to in paragraph 3 should be anonymised in a way that ensures the protection of employees' personal data, in accordance with the provisions of the Act of 10 May 2018 on the protection of personal data, i.e. in particular without addresses, employees' PESEL number). The name of the employee shall not be anonymised. Information such as: the date of conclusion of the contract, the type of employment contract and the length of time worked should be identifiable. 5. The Contracting Authority agrees that the Contractor or subcontractor shall submit a declaration confirming that the persons indicated are employed on the basis of an employment contract. The declaration should include in particular: an indication of the entity submitting the declaration, the date of submission of the declaration, an indication that the activities covered by the request are performed by persons employed on the basis of an employment contract, together with an indication of the number of these persons, the names of these persons, the type of employment contract and the length of time worked, and the signature of the authorised persons
