… to 31 December 2027. 2) The planned average number of hours of services provided per month is 630. 3) The basis for settlements with the Contractor will be 1 (one) working clock hour (60 minutes), which does not include the time of commuting / access to the ward. 4) Services are to be provided on working days, public holidays and non-working days. The number of hours of services provided will depend on the scope of services, the number of charges and their needs and will change during the term of the contract. 5) The Contractor will start providing services to a specific person after receiving from the Ordering Party a catalogue of care services or specialist care services, which will specify the date of commencement of the provision of services, the scope and frequency of activities performed as part of the services. 6) The services are to be provided in accordance with the catalogue of services. In particularly justified cases, when there has been a sudden deterioration in the state of health or when the coverage of services results from the need to urgently provide appropriate care, e.g. after leaving the hospital, it is permissible for the Contractor to start providing services on the basis of a telephone or oral notification made by the Ordering Party. The catalogue of services in writing will be provided immediately at a later date. 7) The service will be ordered on the basis of a work card issued by a social worker employed at the Commune Social Welfare Centre in Ciężkowice. 8) The work card drawn up on the basis of an administrative decision granting such services will contain data: name and surname, contact telephone number (in case of possession) of the person to whom the services will be provided, possibly also the data of the legal guardian, place of provision of services, type of services, scope of care services and number of days per week, number of hours per day, number of hours per month, period for which care services are granted 9) Hours of provision of services do not include the time of arrival of the caregiver to the place of their provision and the time necessary for the movement of the caregiver between the recipients of services 10) Due to the specificity of the subject of the contract, the Ordering Party reserves the right to significantly change the number of persons using care services during the term of the contract and the hourly size of services - without the possibility of claims on this account by the Contractor, among others, in terms of changing the price for the services provided. 11) The Contractor will be obliged to perform the order on the terms and conditions described in the draft provisions of the public contract, which will be introduced into the content of this contract. 12) The Ordering Party shall not be liable for any damage caused by persons providing services on behalf of the Contractor as well as for any damage to the Contractor's employees arising in connection with the provision of services. 13) The Contractor is obliged to keep a book of settlements with the ward from the collected money from the ward, e.g.: for the purchase of food products, industrial products, medicines, etc. The notebook should be found
