… needs, hereinafter referred to as ‘Participants’, who need support in their daily functioning, leaving home, handling official matters or maintaining social activity, without the need to provide permanent care.The services will be provided in full by the PES (Article 2(5) of the Act of 5 August 2022 on Social Economy, Journal of Laws No. Journal of Laws 2025, item 806 as amended). In the case of entrusting the performance of part of the contract to a subcontractor, the subcontractor must meet all requirements as a contractor, including having the status of a social economy entity. Description of the subject of the contract:1)The elderly assistant service will be provided in the municipality of Włoszczowa.2)The elderly assistant service will be provided in the period from 01.04.2027 to 30.04.2029 to persons to whom this service has been granted.3)The said assistance services will be provided in the place of residence of the elderly person in the municipality of Włoszczowa.4)By the hour of service, the contracting authority understands the clock hour provided to the ward.5)The assistance service may be provided 12 hours a day, 7 days a week from 6.00 to 21.00, with the same assistant up to 12 hours a day. 24 hours should be understood as 24 consecutive hours, starting from the hour when the assistant starts providing the assistance service. Until the provision of the assistance service, commuting to and from the participant is not included.6)Services will be provided to the elderly in a variable number from 20 to 40 in one billing month 7)Maximum total number of hours of services in the period from 1 April 2027 to 30 April 2029 16160 hours.
