… Subcontractor, on the basis of the employment contract, of persons performing the activities indicated by the Ordering Party in the scope of contract performance, the Ordering Party may demand, in particular: 1) statements of an employed employee 2) statements of the Contractor or Subcontractor on the employment of an employee on the basis of an employment contract, this statement should contain in particular: the precise identification of the entity submitting the declaration, the date of submission of the declaration, the indication that the activities covered by the request are performed by persons employed under an employment contract, together with an indication of the name and surname, the number of these persons, the type of employment contract, the date of conclusion of the contract, the length of time, the scope of duties and the signature of the person entitled to submit a declaration on behalf of the Contractor or the Subcontractor 3) a certified copy of the employment contract of the employed employee (together with a document regulating the scope of duties, if it has been drawn up) 4) a certificate of the competent branch of the Social Insurance Institution, confirmation of payment by the Contractor or the Subcontractor of social and health insurance contributions for employment under employment contracts for the last settlement period; 5) other documents containing information, including personal data, necessary to verify employment on the basis of an employment contract, in particular the name and surname of the employee, the date of conclusion of the employment contract, the type of employment contract and the scope of the employee's duties.
