Quantity requested: 12 000 pcs Submission of a non-sterile sample The contracting authority requires the submission of a non-sterile sample already within the deadline for submission of the tender. The tenderer is obliged to submit, together with the tender, 1 complete non-sterile sample of the installation set offered for the relevant part of the subject of the contract. The sample submitted must contain all the components required by the technical specification and must be identical to the product offered in terms of composition, material composition, dimensions, functional characteristics and arrangement of the individual components, with the exception of sterility. Deadline and method of submission of the sample: Samples must be delivered: no later than the deadline for the submission of tenders, to the address of the contracting authority, namely the Public Procurement Department, Mgr. Katarína Kramná, entrance hall, block A/0 Directorate, door number 01423104, in closed packaging with the indication: ‘Sample — Introduction sets with accessories according to the requirements for interventional procedures in cardiology and radiology part ....– DO NOT OPEN’. Designation of the sample: Each sample must be clearly marked: - Commercial name of the product, - Name or business name of the tenderer, - Number of the relevant part of the contract, - Marking the item according to the tender documents. The submitted sample will be subject to expert assessment by the Commission in order to verify compliance with the required technical, functional and quality parameters.
