Przedmiotem zamówienia jest świadczenie usługo grupowego ubezpieczenia na życie pracowników oraz członków rodzin pracowników Szpitala Powiatowego w Chrzanowie. W wyniku rozstrzygnięcia niniejszego postępowania przetargowego zostanie zawarta umowa, zgodnie z Załącznikiem nr 4 do SWZ. Wykonawca wystawi polisy określające przedmiot i zakres odpowiedzialności, sumy ubezpieczenia, wysokości składek, a także przekaże Ogólne Warunki Ubezpieczenia mające zastosowanie w sprawach nieuregulowanych w Specyfikacji Istotnych Warunków Zamówienia. Zamawiający nie gwarantuje, że wszyscy zatrudnieni pracownicy oraz członkowie rodzin pracowników Zamawiającego przystąpią do grupowego ubezpieczenia na życie. W ciągu trwania okresu ubezpieczenia mogą wystąpić zmiany w liczbie osób ubezpieczonych. Jeżeli pracownik lub członek rodziny pracownika zrezygnuje z ubezpieczenia, Zamawiający nie jest zobowiązany do opłacania za nich składki ubezpieczeniowej. Szczegółowy opis przedmiotu zamówienia wraz z klauzulami rozszerzającymi zakres ochrony ubezpieczeniowej określa Załącznik nr 1 do SWZ.
