ДЕКЛАРИРАНЕ: Участникът декларира съответствие с поставения критерий с попълване на Част IV, раздел „А“ от ЕЕДОП. ДОКАЗВАНЕ: При условията на чл. 67, ал. 5 от ЗОП или при подписване на договор, документът, който се представя от участника за доказване декларираната в ЕЕДОП информация, е: 1. За чуждестранни лица - в случаите по чл. 112 от ЗОП: Разрешение от изпълнителния директор на ИАЛ за търговия на едро с лекарствени продукти по чл. 195, ал. 2 от ЗЛПХМ, или за внос по чл. 196, ал. 2 от ЗЛПХМ. Възложителят не изисква заверено копие на разрешението или на друг документ, удостоверяващ правото за търговия с лекарствени продукти или разрешение за производство или внос, когато има достъп по служебен път или чрез публичен регистър, или когато същите могат да бъдат осигурени чрез пряк и безплатен достъп до националните бази данни на държавите членки на Европейския съюз.

Съгласно чл. 62, ал. 3 от ЗОП, информацията може да обхваща и по-кратък период в зависимост от датата, на която участникът е създаден или е започнал дейността си. ДЕКЛАРИРАНЕ: При подаване на офертата, участникът декларира съответствие с поставения критерий с попълване на Част IV, раздел Б от ЕЕДОП.
