… conclusion or later accession to the contracts entering into force for the first time on 01.03.2025 are the complete and timely transmission of the following documents (available for download under the link mentioned under 5.1.11.) to the following department at DAK-Gesundheit: 1. Letter of Offer 2. Recognition of the contractual requirements of DAK-Gesundheit, in particular also of the predetermined discounts by submitting the contract with name or company stamp by the interested party at the designated place. 3. Completed Annexes 1 and 2 to the contract. The interested party must enter here the medicinal products for the above-mentioned active substance, which are to be the subject of the contract. The documents must be submitted by e-mail to: [email protected] DAK Health Department Pharmaceuticals (0038 00) Nagelsweg 27-31 20097 Hamburg Annex 2 is expressly to be sent as an Excel file to the e-mail address [email protected]. Contracts under this model and the above-mentioned active substance will be concluded for the first time with effect from 01.03.2025 and will end on 28.02.2027. Interested parties who wish to become contractual partners on this date must submit the above-mentioned documents by e-mail by the 5th of the previous month. The offer must be submitted in text form as an electronic offer PER E-MAIL to [email protected]. This procedure does not provide for the submission of tenders by post. Please DO NOT submit your tender via the German Public Procurement Portal. An electronic receipt of the complete documents PER E-MAIL to the e-mail address [email protected] is sufficient for the deadline to be met. Later accessions are possible at any time. In order to be able to join during the term of the contract on the 1st or 15th of the following month, the above-mentioned documents must be available to DAK Health by the 5th of the previous month, irrespective of the desired accession. Here, too, timely access by e-mail to the DAK-G is essential. Example: The accession requested by the tenderer is scheduled to take place on 15 July. (following month), the documents would have to be submitted by 05.06. (Previous month). Note: Publication is made here using the open procedure form. As already made clear, this is not one - the notice had to be used for technical reasons, as there is no form suitable for approval procedures.
