… human remains and remains (i.e. Journal of Laws 2021, item 1910); 5) the Regulation of the Minister of Internal Affairs and Administration of 1 August 2001 on the method of keeping records of graves (Journal of Laws No. Journal of Laws 2001, No 90, item 1013 as amended); 6) the Regulation of the Minister of Health of 6 December 2001 on the list of infectious diseases for which the declaration of death requires special treatment of corpses of persons who have died from these diseases (Journal of Laws No. Journal of Laws 2001, No 152, item 1742); 7) Resolution No XLVII/431/22 of Dzierżoniów City Council of 28 November 2022 on the regulations of the Municipal Cemetery in Dzierżoniów; Order No 633/2022 of the Mayor of Dzierżoniów of 1 December 2022 on the determination of fees at the Municipal Cemetery in Dzierżoniów. 1.2 The subject of the contract includes: - As regards the administration and maintenance of the Municipal Cemetery, in particular the following duties: 1) running the office of the Municipal Cemetery located in the Cemetery from Monday to Friday from 8.00 to 16.00, on Saturday at the hours declared by the contractor in the submitted offer; 2) keeping records of graves in paper and electronic versions, in accordance with the Regulation of the Minister of Internal Affairs and Administration of 1 August 2001 on the method of keeping records of graves and other necessary documentation related to the administration and maintenance of the cemetery; 3) maintaining a database in the computer program "Interactive Administrator of Cemeteries - Grobonet" and its ongoing operation and updating, immediately informing the Ordering Party and the producer of the program "Interactive Administrator of Cemeteries - Grobonet" about any irregularities in the functioning of the program or other situations preventing or hindering the full use of the program; The Contracting Authority shall make available the Contractor’s computer program; 4) conducting rational management of burial places in accordance with the plan of the Municipal Cemetery; 5) determining the burial places; 6) Entry to the cemetery: a) caravans delivering the dead, b) vehicles providing religious services during funeral ceremonies, c) vehicles with the consent of the administrator; 7) granting consent and allowing entry to the cemetery for vehicles transporting disabled people from Monday to Friday. 8:00-15:00, on Saturday in the hours declared by the Contractor in the submitted tender; 8) issuing a permit for: a) setting up benches, fences, gazons and other devices in the cemetery area, b) performing stonework and construction works, c) planting plants, d) entering vehicles into the cemetery area; 9) supervising the construction of tombs and gravestones in terms of their compliance with the granted permit and the safety of neighboring graves; 10) marking graves in the cemetery that have not been carried over after 20 years; 11) posting on the information board and in the program referred to in § 1 sec. Point 1(3) of the draft contract (Annex 11 to the ToR) , information on graves to be destroyed; 12) carrying out, according to the procedure agreed with the Ordering Party, the liquidation of graves; 13) presentation of a report on the number of graves for which no fee has been paid
