The subject of the contract is the provision of a service consisting in providing access for employees of the NFOŚiGW office and their accompanying persons, students and children to sports activities of a diverse nature as part of a monthly subscription (Sports Package), including for adults at least: gym, swimming pool and fitness centre, for children with a swimming pool card – at least a swimming pool and for children with an extended card – at least a swimming pool, dance and martial arts. … establish cooperation in Warsaw and other Polish cities during the term of this contract. An up-to-date list will always be available on the Contractor's website. If the Contractor provides for identification of users on the basis of identifiers issued by him (e.g. cards), the cost of identifiers (including their delivery to the Ordering Party's headquarters) shall be borne by the Contractor. The identification of the user will take place only by verifying his name and surname, on the basis of the document confirming his identity (e.g. ID card, passport, driving license, and in the case of school-age children, e.g. ID card). The Ordering Party does not allow other forms of verification. Additional information: The ordering party currently employs 800 employees. The Ordering Party, to use the Sports Package in the first settlement period, plans to report the following number of Users: a. 370* Employees / Pensioners of former employees b. 140* Accompanying persons c. 38* Children (swimming pool card) d. 8* Children (extended card) e. 88* Youth (15+ up to and including 26 years of age) * the figures given are the estimated expected numbers of subscribers per month. The Ordering Party reserves the right to report the number of persons before the beginning of each billing period according to current needs. Possibility for the Employee to report as part of the Sports Package: - children (maximum 4), as entitled to use at least pool facilities, - accompanying persons (maximum 2) - young people (maximum 4). The planned co-financing from the Social Committee concerns employees and pensioners (former employees) of the National Fund for Environmental Protection and Water Management (NFOŚiGW) and amounts to: Income up to PLN 4330.00 additional payment of PLN 40.00; II Income up to PLN 6200.00 surcharge PLN 35.00; III Income from 6200,01 surcharge 30,00 zł.
