… performing medical activities (i.e. OJ Journal of Laws 2022, item 402) and the Regulation of the Minister for Health of 5 December 2022 on medical examinations of persons applying for the right to drive vehicles and drivers (Journal of Laws 2022, item 2503 as amended) for employees of the Marshal's Office of the Silesian Voivodeship. 3. Description of the subject of the contract (abbreviated: OPZ) is included in Annex 3 to the ToR/Annex 1 to the Agreement (draft provisions of the Agreement). 4. The draft provisions of the contract relating to the above public contract, which will be incorporated into the text of the contract, are set out in Annex 2 to the ToR. 5. The subject of the contract is in line with the criterion of access to all users, including people with disabilities. In accordance with the provisions of the CIP, the contracting authority requires the provision of medical services in a facility adapted to the needs of persons with disabilities, ensuring: • accessibility of the building, floors and rooms - to the main entrances of buildings serving patients, driveways and exits provide wheelchair mobility, • toilets adapted to the needs of persons with disabilities, • elevators adapted to the needs of persons with disabilities in the case of multi-storey buildings 6. The offer must cover the entire contract, the Ordering Party does not allow the possibility of submitting partial tenders. 7. The subject of the contract will be implemented successively for a period of 24 months, from 1 January 2027 to 31 December 2028 or until the date of exhaustion of the maximum remuneration due to the Contractor, whichever occurs first.
