… commissioning and operation of services, d) lease of optical fibres (dark fibres), e) provision of services cooperating with fixed telephony services provided by the Contractor in support of remote work. 2.2. In the scope of services provided to the Contracting Authority of the District Centre for Family Assistance in Mielec: a) provision of fixed telecommunications services together with the lease of a telephone exchange and the necessary equipment together with the performance of all works related to the commissioning and operation of services, b) provision of services of permanent access to the Internet via symmetrical links and performance of all works related to the commissioning and operation of services. 2.3. In the scope of services provided to the Contracting Authority, the Centre for the Service of Units of the Mielec District: provision of fixed telephony telecommunications services together with the performance of all works related to the commissioning and operation of services. 1. A detailed description of the subject-matter of the contract, including the minimum parameters and requirements of the Ordering Party and the relevant provisions of the contract, is set out in Annex 4 to the FTZ – Model contract and Annex 5 to the FTZ – Description of the subject-matter of the contract. 2. If the subject-matter of the contract includes or may include the ICT , product, ICT service or ICT , process, the Contractor shall only offer ICT products, ICT services and ICT processes used for the performance of the contract, whether as main components or as components, elements of the service environment or associated solutions, which, at the date of submission of tenders: 2.1. are not shown in the recommendation referred to in Art. 4 of the Act of 5 July 2018 on the national cybersecurity system, stating their negative impact on the basic interest of state security; 2.2. do not include an ICT product of the type specified in the decision on the designation of the provider as a high-risk provider referred to in Article 67b(2); 15 of the Act of 5 July 2018 on the national cybersecurity system, nor ICT services or ICT processes specified in this decision. 3. The contracting authority, in accordance with Art. 1 of the Act requires the employment by the Contractor or subcontractor on the basis of an employment contract of all persons performing activities in the field of performance of the subject of the contract consisting in performing the following activities: supervising the performance of the contract, if the performance of these activities consists in performing work in the manner specified in Article 22 § 1 of the Labour Code of 26 June 1974 (Journal of Laws No. Journal of Laws of 2025, item 277 as amended, hereinafter referred to as the Labour Code). The method of verifying the employment of these persons and the Employer's powers to check compliance by the Contractor with the requirements related to the employment of these persons and penalties for non-compliance with these requirements are specified in the model contract constituting Annex 4 to the FTZ.
