… Intensive Care in segment E’. 2. The contracting authority requires the column ‘Proposed lamp type’ to be completed in the Assortment Form (Annex 2 to the FTZ). 3. Failure by the Contractor to fill in the column ‘Proposed lamp type’ in the Assortment Form or to enter ‘No’ or ‘0’ in that column will result in the Contractor’s tender being rejected. 4. The description of the subject of the contract shall include: – Annex 2 to the FTZ – ‘Assortment form’, – Annex 9 to the FTZ – ‘Description of the subject-matter of the contract and its annexes’. 5. Detailed conditions for the performance of the subject-matter of the contract were included in the draft provisions of the contract - Annex 5 to the FTZ.
