1. Przedmiotem zamówienia jest usługa przeprowadzenia zajęć przez rehabilitanta, psychologa i logopedę – usługi specjalistyczne dla 10 osób dorosłych w zakresie rehabilitacji, wsparcia psychologa, logopedy w zależności od potrzeb uczestnika średnio po 6 godzin w miesiącu. Łączna ilość świadczonych usług w wymiarze 1500 godzin zegarowych, realizowane na podstawie ustalonego pomiędzy Zamawiającym a Wykonawcą harmonogramem realizacji usług. Zajęcia będą się odbywać w miejscu zamieszkania osób na terenie Gminy Smętowo Graniczne. 3. Szczegółowy opis przedmiotu zamówienia w tym zakres usług objętych zamówieniem oraz wymagania Zamawiającego określają: SWZ oraz projektowane istotne postanowienia umowy – załącznik nr 5 do SWZ.
