HUS-yhtymä (jatkossa HUS tai tilaaja) pyytää osallistumishakemuksia yksittäisten silmälääkärin vastaanotto- ja tutkimuskäyntien hankinnan dynaamiseen toimittajarekisteriin. Hankinnan kohteen piiriin kuuluvat tilaajan hoitovastuulla oleville avohoitopotilaille hankittavat yksittäiset silmätautien erikoisalaan kuuluvat vastaanotto- ja tutkimuspalvelut (jäljempänä palvelu tai palvelut). Yksittäisiä vastaanotto- ja tutkimuspalveluita ovat pääosin terveyskeskuslääkärin lähetteen perusteella tarvittavat silmälääkärin tarkastukset tai yksittäisen sairauden tai leikkauksen jälkikontrollit. Hankinnan kohde on kuvattu yksityiskohtaisemmin liitteellä 1 Alustava hankinnan kohteen kuvaus sekä alustavassa tarjouspyynnössä ja sen liitteissä. Dynaamisen toimittajarekisterin toimintaperiaatteet on kerrottu tässä osallistumispyynnössä ja sen liitteissä. Ks. liite 2 Menettelyn kuvaus. HUOM.
